Follow the on-screen directions for purchasing a Vitality plan.
Once you have purchased a plan, you will then receive your own membership number. An email will be sent to you asking you to confirm this membership number for the purposes of obtaining the gift card.
You will need to enter your own membership number where prompted on the email. This action needs to be completed within 60 days of the plan start date.
Your plan will then be tracked. It needs to remain active for 90 days before the emails with the gift cards are sent.

On the gift card email, you'll be provided with a choice between an Amazon.co.uk or John Lewis & Partners voucher for £100.

From the date of this email you'll have 90 days in which to choose your gift card redeem the chosen gift card within the same 90 days. Both the gift card and the link will expire after this time.

The John Lewis & Partners gift card can be used in John Lewis or Waitrose shops, or online at johnlewis.com or waitrose.com.

Amazon.co.uk gift cards can only be redeemed online at amazon.co.uk.
