Your acceptance letter is a document which confirms the terms of your cover with Vitality. It will also advise whether there are any exclusions or loadings on your plan.

You can find your acceptance letter on your Member Zone and download a copy of the document. To do this:

Log in to Member Zone.
Click on Life and Log In on the top banner.
Your plan(s) will be displayed. Click on the pink plan number for the plan you wish to view.
Scroll to the bottom and click the dropdown View Your Plan Documents.
Click Download PDF.

You can also find your Acceptance letter through the Vitality UK app. To do this:

Tap the Profile icon in the top left
Tap Update personal details
Tap Edit Life plan details
Tap the Life plan number
Tap View your plan documents
