How do I book a Bluecrest Health Assessment?
Log into your Member Zone account and follow the below steps to book a Bluecrest Health Assessment.
Navigate to Vitality Programme on the dashboard at the top of the screen.
Click Get rewarded.
Scroll to the Bluecrest tile and click it.
Click How to book.
Read through the Compliance information before you book and once you're happy, click the three tickboxes then Go to Bluecrest.
A new browser will open, select the assessment package you'd like to complete.
Select the date and location you'd like for your assessment. There are over 2,000 locations available. You can search using your postcode for the nearest location.
Select the time for your assessment.
Check your details are correct then click CONTINUE.
If there's a problem
They can assist if the booking portal is not working or if you need to rearrange or cancel an appointment. If you do cancel, give Bluecrest 24 hours notice to avoid cancellation fees.
As this a once per plan year benefit, if you miss your appointment, you will not have another assessment available until your plan renewal.
