In some instances, we may ask you and your GP to complete a claim form (Condition Information Request form). This form gives give us additional information from you and your GP so we can determine if we are able to support with your claim.
The form needs to be completed by your usual GP; the Vitality GP will not be able to complete the claim form. This is due to your usual GP having access to your full medical history.
On return of these forms, we will consider the information provided against your plan and the underwriting type you have.
Where can I find my claim form?
If we have informed you that we would need a claim form, we will send it to your Member Zone. You’ll be able to find it in your notifications section.
How do I fill out the form?
There are two parts to the form, the first part is for you to complete and sign, which also gives us permission to talk to your GP. The second part of the form is for the GP to complete. We will need all sections returned to info@vitality.co.uk *or via post to VitalityHealth, PO Box 617, Darlington, DL1 9FF.
How long does it take to make a decision?
We will usually let you know our decision within a maximum of two working days after receiving the information.
